If you don’t see your provider listed below for electronic requests, you will need to download the authorization form. To retrieve your request, you will need the tracking number and the download code you received. Depending on the requested media, payment is due upon submitting a request (starting at $25).

Authorization Form

Completion of the document authorizes the disclosure and/or use of health information about you. Failure to provide all information requested may invalidate this authorization.

Step 1: Download the fillable PDF form.

Step 2: Fill out the entire authorization form, making sure to provide the correct email address or fax number for receiving the medical records.

Step 3: Add your electronic signature to the form and print/save as PDF. Or you may print and physically sign.

Step 4: Send the completed and signed form to one of the following delivery options:

  • Fax to (530) 241-6928

  • Deliver it in person to 2700 Eureka Way, Redding, CA 96001

  • Mail to PO Box 991522, Redding, CA 96099

Please allow 3 business days to process your request. When your records are ready you will receive an email with your tracking number and a link to download.

*If it has been more than 3 days since your initial request, or if you have lost your tracking number, please fill out our check status request.
